If you are unable to use the assessment app or access the Caregiver Portal, accessors can send a generated survey link directly to your inbox. You will need to request a generated link from your child’s assessor.
To complete the caregiver survey with a generated link, follow these steps:
- Contact your child’s assessor and request a generated link.
- Follow the link in the assessor’s email (This will bring you to the caregiver survey).
- Complete the survey questions truthfully and entirely.
- Select ‘Submit’.
