The caregiver survey can be completed multiple times, but should only be completed once per caregiver.
A caregiver can be anyone who has regular interactions with the child in their home life, including their parents, grandparents, older siblings, aunts/uncles, etc.
Completing multiple caregiver surveys is not mandatory, but it does help to provide a more complete picture of each child’s access and opportunities for academic growth.
